PA : Test Score: Title of the Test Score should be Student Name + Test but it shows up as Undefined

During Program Application Process, while filling the Test scores information, On the CRM side the name of the Test Score Record should be Concatenated Text (Student Name + Test Score Name) which would make sense when admin team looks into the record. There is field A-Level Art which is not used in the Test score.
As part of this bug, we have setting the record name to set to student first name + last name + '-' + Name of the Test type when submitted from portal. We are setting this up as it will be clear when looked from the CRM side by the Admin Users on CRM.
Also, we have deprecated/removed the field A-Level Art field on the Test-Score form as it is not being used actively.
